Aw, Jen... I'm so very sorry. It's just so hard after fighting battles for so long. She was obviously dearly loved by her family so I know she had a wonderful life. You were such a blessing to her.

No one can prevent you from testing your dog's blood sugar at home. Just go get a human meter - best bet on brand is a OneTouch if you have those handy there - and do it.

We used to always save a little in the old bottle for situations like this so we could compare and see if there was a problem with a new bottle....

Rotties seem to be challenging diabetics - not sure why exactly - but it is definitely a trend I've seen over the years. They can go a while on a dose of insulin and then abruptly it's far too much...

AAHA Guidelines for diabetes 2017 update

There's some excellent information in these guidelines - including immediately giving Vetsulin / Caninsulin twice a day - and a quite conservative starting dose of 1/8 of a unit per pound.

The U.S. version of Caninsulin (Vetsulin) eventually dropped the emphasis on starting with one injection per day, wisely, and I really wish the Canadian and British guidelines would do the same.

The weak and lethargic behavior worries me that ketones might be present.

The insulin is working - she just has a “mountain” curve instead of a valley. I actually really like mountain curves because lowest blood sugar occurs at mealtime and you don’t have to worry about...

As others have noted, her being unsprayed is a big part of the problem. Rarely, unsprayed female dogs are diabetic only when they are in heat and the blood sugar goes back to normal between cycles.

We actually have had a few members from South Africa over the years.

So at 21kg, she's about 46 pounds so a dose of 12 units of Isophane (NPH) is not...
