Treatment:
(1) IV Isotonic Crystalloid Therapy
(2) Electrolyte Supplementation if potassium, phosphorus, and magnesium are abnormal
(3) Regular Insulin as continuous rate IV or intermittent low-dose intramuscular with a goal of maintaining blood sugar between 200 and 300 mg/dL and drop in BG no faster than 70-100 mg/dL per hour.
Once the patient is hydrated, the route of regular insulin administration can be switched from IM to subQ if desired. If started giving IV continuous infusion, no need to switch to subQ insulin once hydration is good.
If ketones are still present, continue administration of regular insulin.
